One-on-One Community Support is a personalised service under the NDIS that helps you get out into your community, build independence, and feel more connected. At United For Care, we provide the kind of support that makes your day-to-day activities more enjoyable, meaningful, and empowering.
What is One-on-One Community Support?
This service helps you participate in your community in a way that’s aligned with your goals. Whether it’s attending appointments, going shopping, or exploring hobbies, you’ll be supported by a friendly, professional support worker.
Funded under Core Supports – Assistance with Social and Community Participation, this support includes:
- Attending community events and social groups
- Visiting the library, gym, or local shops
- Support with catching public transport
- Going to medical appointments
- Attending faith-based or cultural gatherings
- Exploring interests like music, art, or sport
Why is this important?
Many people with disability experience social isolation. With one-on-one community access, you don’t have to go it alone. You’ll build confidence, learn new skills, and reconnect with the world around you.
How it works
We match you with a support worker based on your personality, preferences, and goals. It’s not just about ticking off tasks—it’s about forming a trusted connection and enjoying shared activities.
Supports are flexible and can be scheduled on weekdays, weekends, or after hours. You decide how your time is spent.
