What is COS?

Coordination of Support

Coordination of Support is a capacity building support, funded to assist you to understand and get the most out of your NDIS plan.

At United for Care, we suggest and connect participants to supports that will meet their needs. We proactively manage provider engagement to ensure you are happy and continually building capacity.

Your team of Support Coordinators

Our support coordinators combine their knowledge and expertise to provide purposeful suggestions for support services tailored to your growth. We work efficiently and with confidence that our recommendation will empower you to pursue your personal goals year-round.

Through regular communication we maintain a deep understanding of your progress and evolving needs. We keep you paired with the best providers for you, and maximise on your journey and support from the NDIA.

Our NDIS access service achieves great outcomes

We support people to understand and complete the NDIS Access Process for free. Core to our approach is seeing the possibilities in each individual and supporting them to reach the appropriate outcome, to date we have had a 100% success rate.

We work with the individual, their formal and informal supports to obtain the appropriate documentation. Most importantly we assist health supports to complete their important section of the access request.

This team is the Gold Standard. The staff have a good handle on comorbidity of cognitive impairment, mental health, substance use and challenging behaviours. It’s what makes them so aspirational. What is Psychosocial Support?

We work with people of all ages, their families, and the community, along with collaborating with various organizations to improve access to community resources and essential services to those who need it.

Who is eligible for Psychosocial and In-Home Support under the NDIS?

The NDIS is accessible to people who have a disability caused by a permanent impairment. The impairment may be intellectual, cognitive, neurological, sensory, physical, or psychosocial.

What is an example of community involvement and or service?

Social and recreation activities can include things like visiting friends and family, participating in active hobbies, or attending local community events like theatre or cultural experiences.

What is Support Coordination?

Support Coordination is a capacity building support, funded to assist you to get the most out of your NDIS plan.

What is a Vertical Village?

A Vertical Village is unique residential model that offers private apartments, dedicated staff, great locations, and tailored support services. This facilitates a strong sense of community and social interaction among residents, allowing for a better and more enjoyable way of life.
